All the ways you benefit from the OnTrak program.
OnTrak is part of your health benefits and is available to eligible members (and independents age 18 or older). Your out-of-pocket expenses are normally limited to your regular co-pays for laboratory tests and medications.
Who’s eligibile:
| If you meet the following criteria, you may be eligible for the OnTrak program: |

Prior diagnosis of substance dependence |
| ›› |
Medically stable—certain conditions may preclude treatment |
| ›› |
Medical history that would benefit from treatment |
Your OnTrak benefits include:

An initial medical evaluation |
| ›› |
Outpatient medical treatment as medically appropriate to address cravings and identify co-occurring conditions |
| ›› |
Motivational Enhancement Therapy and Cognitive Behavioral Therapy (generally three months of treatment) |
| ›› |
A personal Care Coach who supports your efforts and helps enact your care plan for an entire year |
| ›› |
Out-patient treatment keeps you at home with your family, so you can confidentially maintain your routine |
