A custom team of physicians, therapists and coaches are available to address your unique needs. The OnTrak team membes can work with your existing treatment providers to help you meet your goals.

Your OnTrak team uses each of the three program components to help you achieve better health, develop the right skills to maintain your health and provide long-term support.

As a core part of your program, you may have a personal Care Coach for up to an entire year. Care Coaches are an important part of your treatment team, working hand-in-hand with your doctor and therapist to deliver a personalized care plan just for you.
