OnTrak Program Overview
Substance dependence is a disease. OnTrak treats it as such.

Your health plan now offers OnTrak because many people can't beat substance dependence on their own. This new, confidential, outpatient program includes a professional team with years of firsthand experience treating the disease of substance dependence.

The OnTrak program places you under the care of three specialized professionals:
Your customized care plan builds a specially trained team including:

›› Medical doctor with substance dependence expertise,
›› Relapse prevention therapist, and a
›› Personal Care Coach.

Medically treating your substance dependence and reducing your cravings and anxiety will allow you to be an active participant in therapy, learning and developing the skills you need to stay in recovery. Your personal Care Coach provides one-on-one support to help you practice those skills to get you on track to a better life.

OnTrak is convenient.
All treatment in the OnTrak program is performed on an outpatient basis, to minimize time away from family and work.

OnTrak is confidential.
While enrolled in the OnTrak program, your information is shared only with your written consent and only between your treatment providers. OnTrak complies with all state and federal laws established for the protection of your confidentiality.

OnTrak is free.
OnTrak medical visits, Relapse Prevention Therapy and Care Coaching services are all provided at no cost to you. Lab work, prescription medication and other medical care fall under your normal co-payments/coinsurance/deductible.
